This is the form to complete to register with any of the Bluegrass teams. This needs to be filled out and turned in to the team manager.

Soccer Premier is now handling all spiritwear orders. This is the order form for the Spring 2008 Season. Limited quantities will be available at the Soccer Premier store in Lexington.
